Description
A business acquaintance mailed me this image and asked what it was. It has his grandmother's name on it. I understand the Latin text but not the other. Is this Slovak or transliterated Old Church Slavonic or Rusyn? TIA!

I think it is Slovak "Souvenir of First Holy Communion"

Looks like Slovak to me, to the best of my knowledge (i.e. 1.5 years so far of learning/speaking Slovak while living here) it says:
Memorial of First Holy Communion
Who received: Maria Kuchar
The 25th day of May 1930 in the Church of St. Joseph, etc.

(My wife has verified that I am correct... she's a Slovak...)

I'm not sure of the exact Slavic language it's in, but it's similar enough to Bulgarian (which I speak) that I'm guessing it means "In memory of First Holy Reception", ie, Communion, and the chalice in the photo would bear that meaning out. Perhaps others can confirm this, or correct me.

It's Slovak, and means:

Remebrance of First Holy Communion

Now I just have to decide if I want to leave you with impression I know Slovak, or if I should admit that I got that by typing the text into Google Translate...

Slovak, or as my great grandfather would say "REAL Slovak".

"Remembrance of First Holy Communion"

Translated from Slovak.

Apparently from St. Joseph's Church in Hazleton, PA.

Could this be a First Holy Communion certificate?

St Joseph's was one of two Slovak Latin parishes in Hazleton (the other, Holy Trinity, being its daughter) - which, like many cities and towns in the Scranton Diocese, was awash in national parishes (German, Italian, Lithuanian, Magyar, Polish, Slovak, and Tyrolean or Tyrolese - don't see the latter every day).

St Joseph's (1882) is the oldest Slovak Latin Catholic church in the Western Hemisphere. About 2 years ago, Holy Trinity was merged back into St Joseph's, as was St Ladislaus Magyar Church shortly thereafter. The consolidated parish, linked also with St Stanislaus Polish Church, is known as Ss Cyril & Methodius Parish, and worships at St Joseph's.

St Joseph's Church

The priest who signed the certificate was Father Leo J. Kroner, of blessed memory. He was the pastor of St Joseph's from 1918 to 1934, after which he served other Slovak parishes in the area before retiring in 1957. He reposed later that year.

Many years,

Neil
